Psychiatric Advance Directive

📋 Your voice, in advance

A Psychiatric Advance Directive (PAD) is a document that records your treatment preferences for a future mental health crisis — when you may not be able to speak for yourself. It ensures your wishes are known and respected.

Your PAD lets you specify:
✓  Which treatments you consent to — or refuse
✓  Which medications have worked, and which have not
✓  Who should be contacted in a crisis
✓  Where you prefer to receive care
✓  What helps you feel safe and calm
✓  Any cultural, religious, or personal preferences
How it works — 4 steps:
1
Complete the PAD form
Answer questions about your preferences, contacts, and care history. Save and return anytime.
2
Name one or more Trusted Witnesses
Choose a family member, friend, peer specialist, or anyone you trust. They will co-sign your directive and be available in a crisis.
3
Witness endorses the PAD
Your witness reviews the document and confirms: "I have read this directive and agree to serve as a trusted witness and crisis contact." The PAD is not complete until they endorse it.
4
Your PAD is stored and protected
Your directive lives securely in your GAIN profile. You control who can see it. If a crisis alert fires, your caseworker and witness are both notified with immediate access.
⚠️ Note: GAIN will work with legal and clinical partners to ensure PADs meet New York State Mental Hygiene Law requirements. A peer specialist or caseworker can assist you. This feature is currently in development.
